1. Waiver of Liability:

I, the undersigned, as the parent or legal guardian of the child(ren) listed above, hereby acknowledge and agree to the following terms regarding my child's participation in the Traditional Martial Arts (TMA) Day Camp:

• I fully understand that participation in martial arts and Day camp activities involves physical activity that may carry inherent risks, including but not limited to injury or accidents.

• I accept full responsibility for the risks associated with my child's participation in these activities.

• I agree to release and hold harmless TMA, its owners, instructors, staff, and volunteers from any and all liability arising out of or in connection with any injury, accident, or illness that may occur during the course of my child’s participation in camp activities.

By signing below, I waive any right to hold TMA or its staff legally responsible for any incidents, injuries, or accidents that occur during camp or related activities.

2. Emergency Medical Treatment Authorization:

In the event of an emergency, I give permission for TMA staff to seek immediate medical treatment for my child at the nearest hospital or medical facility if necessary. I understand that I will be contacted as soon as possible in the event of an emergency.

3. Permission to Leave the School:

I, the undersigned, give permission for my child to leave the TMA campus for scheduled field trips or events as outlined in the camp schedule.

• I understand that my child may be transported by TMA staff or approved drivers to off-site locations.

• I agree that TMA is not responsible for my child’s actions while off-campus and that my child will be under the supervision of TMA staff during all activities.

I further acknowledge that my child will only be released to authorized individuals, and I must notify TMA in writing if there are any changes to the approved pick-up list.
